Join us for the fifth annual Cooper's Cause Benefit Golf Tournament. Monies collected will help families that have health insurance but do not qualify for many of the programs designed to aid in the unexpected and substantial financial burden related to pediatric heart conditions.

Registration Fee:

$125 per individual
$500 per foursome
Early Bird Special: Just $100 per person for fees paid by June 8, 2012

You may register for the Benefit Golf Tournament online or by filling out and mailing in the registration form.

Lunch and Drinks
There will be a box lunch served prior to tee time and free beverages on the course. An awards banquet and dinner will follow at the clubhouse with a cash bar after the event.

TOURNAMENT SPONSORSHIP

Event Sponsor ($1,500): Includes lead in all advertising and signage on the course day of event, 1st team entry fees waived, complimentary foursome play card at Alvamar Public Golf Course (cart must be purchased).

Hole Sponsor ($250): Includes course signage on day of event, and complimentary foursome play card at Alvamar Public Golf Course (cart must be purchased).
